Administering surfactant without intubation - what does the laryngeal mask offer us?

چکیده

Em nosso manejo de neonatos prematuros que necessitam de apoio respiratório, atualmente lutamos contra prioridades concorrentes. Evitar intubação no início da vida (exceto em casos de reanimação avançada) parece ser uma proposta sensível, tanto em neonatos ≤ 29 semanas de gestação1 quanto certamente naqueles acima de 29 semanas. Por outro lado, evitar intubação significa abandonar a conduta habitual de administração de surfactante, o medicamento em si, que, ao longo dos anos, tem sido nosso cobertor de segurança ao lidar com a síndrome do desconforto respiratório (SDR). A combinação de pressão positiva contínua nas vias aéreas (CPAP) com a administração seletiva de surfactante em um pacote de cuidados minimamente invasivos representa uma grande promessa para neonatos prematuros de todas as gestações,2,3 evita armadilhas na ventilação mecânica, independentemente de quão bem aplicada. Resumidamente, é necessária uma abordagem em que possamos reconhecer, em estágio inicial, os neonatos submetidos ao CPAP que têm SDR significativa e administrar seletivamente surfactante nos neonatos sem recorrer
